I hereby authorize LaFontaine Christian Church and its affiliated ministries to publish photographs, videos, and audio recordings of the undersigned minor children for use in LaFontaine Christian Church and its affiliated ministries’ printed publications, marketing materials, websites, and/or social media.

I release LaFontaine Christian Church from any expectation of confidentiality for the undersigned minor children and attest that I am the parent or legal guardian of the children listed below, and that I have the authority to authorize LaFontaine Christian Church to use their photographs, videos, and names.

I acknowledge that since participation in LaFontaine Christian Church printed publications, marketing materials, videos, social media, and websites is voluntary, neither the minor children nor I will receive financial compensation.

I further agree that participation in any publication, video, digital or social media content, or website produced by LaFontaine Christian Church confers no rights of ownership whatsoever.

I release LaFontaine Christian Church and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.

I irrevocably give consent to LaFontaine Christian Church, its employees, and its assigns to make use of my image and the images, likenesses, appearances, and voices of the undersigned minor children in photographs, videos, and sound recordings.

I acknowledge that I am fully aware of the contents of this release and that I am under no disability, duress, or undue influence at the time of signing this instrument.
